For your first visit, please arrive 15 minutes early to provide adequate time to sort out any formalities. New patients can save time during their initial appointment by completing a Patient Information Form prior to their visit. Simply print it out, fill in the information requested, and bring the completed form with you to your appointment.
If you are a new patient and having an online consultation, please download the form. Once you have filled it out, please email the form (preferably as a pdf or doc) as soon as possible to office@health-care.pl
We offer direct insurance billing with selected global insurers: please bring your insurance card and check with our reception.
Payment is due at the time of service.
