Dear Patients,
This is an update about current COVID-19 vaccinations’ schedule (note: may be changed by the government at any time).
At the moment only patients over 60 years old are being vaccinated. Patients above 40 years old, who had earlier registered their interest in being vaccinated (through government site www.szczepimysie.org.pl) should already been issued automated referrals. Those patients can book their vaccination dates as follows:
– if you are between 40 and 49 years old you can book a vaccination date after 15th May 2021
– if you are between 50 and 59 years old you can book a vaccination date even before 15th May 2021.
All patients with a PESEL number who had not earlier registered their interest in being vaccinated through www.szczepimysie.org.pl will automatically be issued referrals according to the schedule below:
– people born in 1962 – from 12th April 2021
– people born in 1963 – from 13th April 2021
– people born in 1964 – from 14th April 2021
– people born in 1965 – from 15th April 2021
– people born in 1966 – from 16th April 2021
– people born in 1967 – from 17th April 2021
– people born in 1968 – from 19th April 2021
– people born in 1969 – from 20th April 2021
– people born in 1970 – from 21st April 2021
– people born in 1971 – from 22nd April 2021
– people born in 1972 – from 23rd April 2021
– people born in 1973 – from 24th April 2021
Patients without a PESEL number need to book an appointment with a doctor in order to generate a referral for them. For logistical and legal reasons we are unable to generate this referral without a doctor’s consultation. Once a referral is issued you will need to book a vaccination date through one of the following ways:
– online at www.pacjent.gov.pl
– by phone using a dedicated information line 989
– directly in vaccination centre
Please remember that you will not be able to book a vaccination date without a referral.
If you are one of our patients and are eligible for a vaccine please send us an email to office@health-care.pl with your name and surname, date of birth, PESEL number (if you have one), passport number and date and place of issue (if you not NOT have a PESEL), contact phone number and your address in Poland.
